By submitting any material (i.e., photographs and video recordings) for posting on MDA’s web sites, I, on behalf of me/my minor children, consent to Muscular Dystrophy Association, Inc. ("MDA"), its officers, directors, employees, agents, chapters, and representatives, to use my/my children’s name(s), picture(s), portrait(s), likeness(es), writings, biographical information, and photographs, audiotape recordings, and/or videotape recordings of me/my children and my/our real and/or personal property in any medium for editorial, educational, and promotional purposes, for the solicitation of contributions, and for any other purpose in furtherance of the charitable mission of MDA. I understand that MDA retains full editorial control regarding all material submitted for posting or publication.

I represent that (i) I am 18 years of age or older, (ii) I am the owner of the submitted material, or I am submitting such material with the express consent of its owner, and (iii) I have the permission of others who have contributed to or are featured in any material that I submit and, if there are any individuals under the age of 18 in any such material, I have obtained the permission of each such individual's parent/legal guardian prior to submitting such material.

By submitting any material, I also agree to abide by the terms of MDA's User Agreement and Privacy Policy.

This consent shall be binding upon me/my minor children and our respective heirs, executors, administrators, and assigns.
