MDA's Transitions Center will feature one person each week with a neuromuscular disease who has demonstrated that living with a disability is only ONE part of his/her story. You can nominate anyone between the ages of 18-40 who has a neuromuscular disease. Please complete each section of the form below and submit a photo and/or video with the nomination. Thank you.
